To become a member, you must complete the “Application for Membership” form and return it to us along with a $10 deposit. Please be certain to provide information in all areas requested, and to sign your name in all areas indicated. If there are to be joint owner(s), have them complete the Joint Owner section(s), and sign by the areas indicated. We must have all of this information before we can open the account.

PLEASE SEND LEGIBLE PHOTOCOPY OF TWO FORMS OF ID ON ALL PARTIES TO BE ON THE ACCOUNT. You must also have the Notary form completed if you do not return the forms in person.

The following is a list of acceptable ID. The ID provided must show your name, Social Security/Tax Identification number, residence, and date of birth. ONE FORM OF ID MUST BE A PHOTO ID LIKE:

• Un-expired state drivers license Military ID card
• Passport Immigration Card
• Any other form of Government Issued ID Employer issued photo ID *(1)
• Student ID card *(2)

Other identification documents can be:

• Social Security Card Certified Birth Certificate
• Pay Stubs Immigration Documents
• Un-expired employment authorization documents
• Utility Bills

Individuals opening accounts for minor children will have to provide ID for themselves and a Social Security Card and Birth Certificate for the minor child.
